  1. Members simply need to  download and complete the Choice of Fund form
  2. Then send the form to your new employer

  1. Simply download the ‘It’s your choice’ form.
  2. Insert your name, smartMonday member number and the Product details (you can find this information by logging into your account and navigating to the ‘Account & contacts’ section. Refer below images).
